The Family Wellness Program was established in 2000 to build healthier communities through awareness, knowledge, and empowerment of individuals and families. We specialize in serving those at risk for or diagnosed with diabetes and its associated comorbidities.

Family Wellness Center Director: Elva Hooker
Elva is a proud registered dietitian, certified diabetes educator, but her favorite title is to be called mom by her son Logan. She is a master trainer for the National Diabetes Prevention Program and on her spare time she loves to spend time with her son, husband, and enjoy a good book.
Registered Dietitian Nutritionist: Monica Diaz
Monica received her Bachelors of Science in Nutrition and Dietetics from Arizona State University and completed her Dietetic Internship at Iowa State University. Monica believes that prevention through our everyday habits is key to living a healthy life! On her spare time she likes to cook and watch Disney movies.
Registered Dietitian and Certified Diabetes Educator: Maria Silva
Maria is currently working to obtain her masters in Healthcare administration and enjoys promoting a culture of philanthropy and success. In her spare time she likes to cook, eat, and dance.
Registered Dietitian and Certified Diabetes Educator: Elvia Lish
Elvia received her Bachelor of Science degree from Arizona State University and completed her dietetic internship, through Iowa State University. Her passion for serving transcends the walls of St. Vincent de Paul as she serves as an active member of the executive team of the Academy of Nutrition and Dietetics. In her spare time she loves to travel, spends time with her husband Brandon and two beautiful and active huskies.
Health Coordinator: Maria Gonzalez
Maria holds a Bachelor of Science in Human Nutrition from Arizona State University and soon will be applying to medical school. She enjoys powerlifting and watching action movies. Maria believes that by providing quality nutrition education to the community she serves, she will make an impact on many individual’s well-being.
Health Educator: Mariana Rodriguez
Mariana is soon to graduate Arizona State University with a bachelor in nutrition, and soon after dive into the journey of becoming a registered dietitian. She is a firm believer that nutrition education empowers individuals to take control over their health and wellbeing.
