The Society of Health and Wellness for the Whole Family

According to the CDC, 1 out of every 11 people have diabetes and more than 1 out of every 3 have prediabetes. 

Help fund our community outreach and health education programs. 

The Family Wellness Program was established in 2000 to build healthier communities through awareness, knowledge, and empowerment of individuals and families. We specialize in serving those at risk for or diagnosed with diabetes and its associated comorbidities. 

 

Family Wellness Program services include: 

Diabetes Self-Management Education: Adults diagnosed with diabetes participate in a series of classes that provide individuals with the knowledge and tools to gain diabetes control, live healthy, and delay or prevent the onset of complications.

Every Little Step Counts: Families with children between the ages of 8-16 at risk for diabetes participate in a series of classes that help build positive lifestyle behaviors for the whole family. The program experience is enhanced with the support of our community partners. A similar program is also available to adults.

Medical Nutrition Therapy: Participants needing more intensive nutrition therapy are provided with individual consultations for further management by the Certified Diabetes Educators and a Registered Dietitian Nutritionist.
